Why is the concept of the start menu so hard for some people to understand?

A (l)user just called to tell me Excel was no longer installed on her computer. A tech went and looked and found it on the start menu. The (l)user said "Oh I never use that, is that where all my programs are? I thought they were all on my hard drive (pointing at her monitor)

Do you get this a lot too?
Luser: I don't have a Start button.
Tech: The button on the lower left where you click to find your programs?
Luser: Oh, I never noticed that it said Start on it before!
