Our X-Box OS has a great error when it can't find a workgroup on the network:

In PVCS version 5.0, the toolkit used had a few standard routines for messages: error, warning, information. There was a bug where occasionally the wrong one would be used.

I got many calls from users confused about "Error: The Operation Completed Successfully."
